If you are requesting a withdrawal from your investment account, DMA requires a Withdrawal Form to be completed and submitted to vault@dma.co using the appropriate V-Code.
Completing a Withdrawal Form:
- Select if you want the account to be closed post the withdrawal.
- Indicate the DMA account number from which the funds need to be withdrawn.
- Indicate the name of the DMA account holder.
- Name of receiving bank.
- Address of receiving bank.
- SWIFT code of receiving bank. Please note this can be found on your bank's website.
- Clearing code of your receiving bank.
- Receiving bank's branch name.
- Amount to be withdrawn.
- Currency of amount to be withdrawn.
- Account number or IBAN of the receiving bank account.
- Reference for the payment.
- Corresponding bank's name.
- Corresponding bank's address.
- Corresponding SWIFT.
